This transition guide is designed to help young men with disabilities prepare for adulthood by exploring important life skills, goals, and future planning topics. Using an engaging and relatable format, the book encourages youth to think about education, employment, independence, relationships, self-advocacy, and community involvement as they plan for life after high school.
The resource also supports conversations between students, families, educators, and support teams about transition planning and personal growth. Through activities, guidance, and real-world topics, the book helps young people build confidence, better understand their strengths and interests, and begin making informed decisions about their future.
