Transition Guide: Self-Directed IEP

This transition guide helps students with disabilities take a more active role in leading and participating in their own IEP meetings. It explains what self-directed IEPs are and encourages students to learn about their goals, strengths, support needs, and educational plans so they can communicate more confidently during the transition planning process.

The resource is designed to help students, families, educators, and support teams promote self-advocacy, independence, and student involvement in decision-making. By encouraging youth to speak up, set goals, and help guide discussions about their future, the guide supports greater confidence and preparation for adulthood, education, employment, and independent living.
